HOW DO I CONFIRM MY INSURANCE COVERAGE?
We recommend calling the member services number on the back of your insurance card. Specifically ask about your 'outpatient physical therapy' benefits, including any deductible, co-insurance, or co-pay requirements.
WHAT IF YOU ARE OUT-OF-NETWORK WITH MY PLAN?
If we are not in-network with your specific provider, you may still be able to use your out-of-network benefits. We can provide a comprehensive superbill for you to submit to your insurance company for potential reimbursement.
WHEN IS PAYMENT DUE?
Co-pays and session fees are due at the time of service. We accept all major credit cards, HSA, and FSA accounts for your convenience.
